Repeat Prescriptions

You can request your prescription in the following ways:

• By placing a Repeat Prescription Form (i.e. the white printed tear off slip on the right hand side of your prescription) into the repeat prescribing box in reception
• By post (please provide a stamped self-addressed envelope if you would like your prescription to be posted to you).
• By asking for a Repeat Prescription Form at the front desk or downloading it below.
• By Fax on 0844 815 1880

NOTE: We only take telephone requests from housebound patients. This is for reasons of patient safety.

Please ensure that the items you require are requested clearly by ticking the appropriate box on the repeat request form, or if you do not have a repeat request form, please ensure that your name, address and the items you require are clearly indicated in the following Request Prescription Form.

Local Chemists

You can also arrange for your local chemist to request and collect prescription on your behalf. Should you require this service, please go to your local pharmacy and ask for assistance.
